Membership
DPC physicians work only for their patients, not for insurance companies. We keep our membership fee low by eliminating the overhead cost associated with billing insurance. The membership fee may be increased annually by an amount no greater than 4%.
There is a one-time registration fee of $100 per person (maximum $250 per family)
$54 per Month - Ages 18 - 39
$66 per Month - Ages 40 & Up
$162 per Month - Families
Includes up to two children, living at home, under age 26. Each additional child $27
$16 per Month - Students
Living out-of-area, with at least one parent being a member
Services Included
Unlimited office visits
Virtual visits by phone or secure video
In-office tests such as EKG, vision screening, hearing screening, urinalysis
Annual wellness visit, including specific goals for improving health
Coordination of care with specialists
Management of chronic medical conditions (Hypertension, Diabetes, High Cholesterol, Asthma/COPD, Arthritis, GERD, Depression/Anxiety, Thyroid Disease, and many others)
